Responsibility
Virginia’s long term care providers serve a key role in the Commonwealth’s safety net for our most vulnerable citizens. Through both good times and tough economic periods, our member facilities take their responsibility to heart and deliver care to those who often have exhausted their options for receiving the services they need. Cutting Medicaid payment rates that already fail to cover operating costs is not an acceptable strategy for Virginia’s policymakers to employ in their quest to balance the state’s budget. Please join us in telling our Governor and Legislators to take their responsibility seriously and provide adequate funding for long term care.

- The downloadable PDF transfer form is interactive. To use the interactive form follow these instructions:
1. Download the form from the Internet.
2. Choose Save As and save to local or network drive of computer.
3. Fill out the form and then save the file as another name (probably with a title including patient's name or other appropriate means of patient identification) so that the original file that was downloaded remains blank for future use.
4. Print the form or forward the Saved copy.
Alternatively, the form can be downloaded, saved, printed and manually completed as needed.
