To provide basic emergency planning, response and recovery guidelines for extended care facilities (e.g., nursing homes, assisting living facilities, adult day care, mental health/retardation facilities). Pre-planning efforts should include developing comprehensive checklists that will assist facility personnel in responding to an emergency. Emergency response guidelines should be tailored to meet specific facility needs. They should also be periodically reviewed and revised by personnel responsible for their use in order to keep the documents current.

To provide guidance in the development of an evacuation plan containing detailed information, instructions, and procedures that can be engaged in any emergency situation necessitating either a full or partial evacuation of the nursing home. This plan must incorporate staff roles and responsibilities essential to this process. Staff must be educated in their role(s). Drills and reviews must be conducted to ensure that the plan is workable. The plan must include back up measures for all components.

As a result of 2006 legislation removing moral turpitude as a
barrier crime to employment in facilities, the Department of Health
Professions and the Department of Health have revised their guidance
document, "Impact of Criminal Convictions on Nursing Licensure or
Certification and Employment in Virginia.
- Nursing Home Administrator Licensure and AIT Requirements (posted 11-29-05)
Nursing Home Administrators are licensed by the Virginia Department
of Health Professions and regulated by the Board of Long Term Care
Administrators. Regulations on qualifications for licensure of
administrators, training facilities and preceptors can be
found here.

- Advanced Nurse Aide Program (posted 11-29-05)
The Board of Nursing published final regulations for Advanced
Certification for Nurse Aides that became effective on April 9,
2003. The regulations contain the qualifications for advanced
certification, requirements for renewal and criteria for approval of
education and training programs.
VHCA has developed a model program for the 120 hour program in
required content areas of leadership and mentoring skills, care of
the cognitively impaired client, restorative care and wound care.
The program has been approved by the Board of Nursing and is
available to members. To request a copy of the entire program, which
includes curriculum content, objectives, sample calendar schedules,

- 2005 Virginia NF DON Turnover Study (posted 10-6-05)
Each year, half of all DONs quit their jobs. In 2002, Virginia
suffered a 142.7 percent DON turnover, the highest rate in the
nation. A coalition led by the Virginia Health Care Association
requested My InnerView to facilitate a study of this issue to
identify and analyze the root causes of the DON turnover in Virginia
and to communicate findings and recommendations to policy makers,
providers and educators.

Five-Star Rating System
Simply stated, the Centers for Medicare & Services
(CMS) imposes on the entire industry a state by state quota system
for the allocation of stars that bears no relation to how a
particular state’s nursing homes might cluster along a continuum.
CMS allocates stars for health inspections as follows: 5 stars-10%;
4 stars-23.3%; 3 stars-23.3%; 2 stars-23.3%; 1 star-20%.

Effective December 27, 2005, SNFs and NFs must post information on
staffing on each shift on a daily basis in a prominent place. Other
data that must be posted include the facility name, current date and
resident census. Records must be maintained for 18 months, which
covers the annual survey period and allows surveyors to review the
records if needed.

A fact sheet on Group A Streptococcal Infections prepared by
Elizabeth Barrett, D.M.D., M.S.P.H., Assistant State Epidemiologist
with the Virginia Department of Health, describes the group A
streptococcus, how infections are spread, who is at risk, and what
can be done to help prevent invasive group A streptococcal
infections.
- DDNR Final Regulations (posted 3-5-02)
Final regulations governing the Durable Do Not Resuscitate program
(DDNR) become effective March 27, 2002. Issued by the Virginia
Department of Health, they replace the Emergency Medical Services Do
Not Resuscitate Order regulations currently in effect. As published
in the February 25, 2002 Virginia Register, the regulations finalize
requirements for a Durable Do Not Resuscitate (DDNR) Order that
follows the patient throughout the entire health care setting. Once
issued by a physician for the patient, the DDNR Order would apply
wherever the patient may be - home, emergency vehicle, assisted
living facility, nursing facility or hospital. According to the
regulations, an original DDNR Order or an alternate form remains
valid until revoked. Authorized Durable DNR Forms, with
instructions, are available only to physicians. The form should be
maintained and displayed at the patient's current location or
residence in one of the places designated on the form or should
accompany the patient if traveling.
While there is no specific guidance on how to obtain them in the
regulations, the Board of Health authorizes the issuance of
alternate forms of identification in conjunction with the issuance
of DDNR Orders. The alternate identification forms consist of
"uniquely designed" and "uniquely-identifiable" bracelets and
necklaces available from a vendor approved by the Virginia
Department of Health. Regulations describe DNR orders and
implementation procedures for the issuance of a Durable Do Not
Resuscitate Order. A sample DDNR Form is included with the final
regulations.
