From a pharmacy perspective, Pruitt Health Pharmacy Services’ Jessica Patton, Pharm.D. will review key strategies for ensuring continuity of care among healthcare settings, reducing rehospitalization risks, and optimizing medication therapy while minimizing costs

Dr. Jessica Patton is a consultant pharmacist for post-acute rehabilitation, skilled nursing, and assisted living centers with PruittHealth Pharmacy Services in the Lowcountry of South Carolina. She also functions as a clinical pharmacy resource for multiple PruittHealth hospice locations that service home care and long term care center patients over four states in the Southeast. She received her Doctorate of Pharmacy from the South Carolina College of Pharmacy at the Medical University of South Carolina in Charleston, SC. In addition, she serves as a preceptor for Advanced Practice Long Term Care student rotations for the South Carolina College of Pharmacy.
