VA Premier: Dedicated Fax Line for DMAS 80 Forms, Phone/Email for Claims Issues
Virginia Premier has asked VHCA-VCAL remind members that it has set up a dedicated fax line to receive DMAS 80 forms and a dedicated phone line and email for SNF/NF claims questions.
The dedicated fax line for the DMAS 80 forms is (800) 846-4254.
The dedicated phone line for SNF/NF claims questions and assistance is (877) 719-7358 ext. 58217. You may also send questions to vphpltssclaims@virginiapremier.com.
