We have reactivated the Issue Log first set up in 2015 to help track and resolve member problems with the Commonwealth Coordinated Care (CCC) Plus program.  You can use this webform to share feedback on issues from eligibility, care coordination, audit/appeals, transportation, etc.  To comply with HIPPA, please do not include any personal health information (PHI) when using the Issue Log. 

If you have not discussed the issue(s) reported with the relevant managed care plan, please do not delay, if appropriate.  We will use the information you provide to seek resolution from the managed care plan and/or DMAS for any of the issues identified. 

If you submit an issue using the Issue Log, when VHCA-VCAL receives feedback from DMAS or the managed care plans, staff will be back in touch with you.  If you receive feedback directly from DMAS or the managed care plan, please email Steve Ford with that information, as other members may also benefit.  If you would prefer to speak with someone at VHCA-VCAL directly, please call 804.353.9101.
