An issue has been identified with MDS 3.0 assessment records submitted to the ASAP (Assessment Submission and Processing) system beginning October 1, 2016, through November 4, 2016. This issue impacts all nursing and swing bed (NO/SO) OMRA assessment records with a target date on or after October 1, 2016, and may have resulted in an incorrect ASAP system-calculated RUG code for these assessments. No other assessment record types were affected by this issue.

According to state RAI/OASIS Coordinator Cil Bullard, RN,CPC, RAC-CT, if you did a stand-alone OMRA during the dates specified, you will need to do a modification of that assessment. So even if you are a swing-bed unit, if you did a plain EOT, COT, or SOT you need to submit a modification. If your OMRA was combined (for instance, an EOT/30d MDS), then you will not need to modify the OMRA.”  Click here for a related memo from the Quality Technical Support Office (QTSO).
