• Submit a picture and profile for our Faces & Stories Initiative to help us show residents receiving quality and loving care at member centers.
• Nominations are due by November 15, 2024.

• To comply with HIPPA requirements, we will need a signed Image Release Form for each individual captured in a photo.
• We will feature the 2025 honorees on our website and on social media.
