Important notes

• Review the nomination flyer for details on preparing a great submission.
• To comply with HIPPA requirements, we will need a signed Image Release Form for each individual captured in a photo.
• We will feature our 2023 honorees on our website and on social media. We will not be producing a printed calendar.
