The deadline for submitting your nominations for the 2022 Faces & Stories in Long Term Care calendar has been extended to October 11. Share a brief story about one of your residents to help us show the compassionate, quality care you provide and how long term care is a critical component of the care continuum.

We will select up to 24 residents to portray in the coming year, on our website, and on social media. Details on how to submit your photos and story can be found in the Faces and Stories nomination package or via our online form.

Please note that to comply with HIPPA requirements, we will need a signed Image Release Form for each individual captured in a photo.
