Trends in Most Frequently Cited Violations

As part of the VCAL Assisted Living Webinar Series held over the past week, DSS Ivy Burnham shared the most frequently cited ALF citations. Of note, there are still consistent trends in the most frequently cited violations related to ISPs and medication administration.

Over the past two years, these items were most frequently cited:

  • 22 VAC 40-73-450 C–Comprehensive individualized service plan completed within 30 days after admission
  • 22 VAC 40-73-680 D—Medications administered in accordance with physician’s orders or other prescriber instructions
  • 22 VAC 40-73-640 A—Current written plan for medication management addressing procedures for administrating medication
  • 22 VAC 40-73-320 A—Within 30 days preceding admission, person shall have a physical exam.

During the webinar Mary Chiles with Chiles Healthcare Consulting, LLC, offered operational best practices and advice on addressing these deficiencies following Burnham’s presentation.

She suggested a focus on these clinical and operational opportunities:

  1. Medication Administration / Management
  2. Individual Service Plans
  3. Resident Safety
  4. Resident Record Management
  5. Incident Reporting Requirements
