The Department of Social Services (DSS) has revised the Assisted Living Facility (ALFs) Disclosure Statement. The disclosure statement has been revised to add a line for the initials of the prospective resident or his legal representative at the bottom of each page of the form.

The instructions for completing the form have also been revised to include an item stating that the prospective resident or his legal representative must initial at the bottom of each page.

DSS advises ALFs to begin using the revised form immediately. Questions about the changes should be directed to your licensing inspector.

Revised Assisted Living Facility Disclosure Statement

Of note, on March 11 DSS sent a message to ALFs about the updated disclosure statement that stated the attachment may contain malware. That message should be disregarded as the message was valid. DSS apologized for the confusion.
