Top 10 ALF Deficiencies: July – September 2017

The most cited deficiencies in Virginia’s assisted living facilities (ALF) for the fourth consecutive quarter are again related to Individualized Service Plans (ISP) and medication administration. 

Of the 318 facilities inspected from July 1 – September 30, 2017, the Department of Social Services (DSS) cited the following:

  • 49 deficiencies cited under 22VAC40-72-(6)-440-C: Individualized service plans – reflecting assessed needs, and
  • 36 deficiencies cited under 22VAC40-72-(6)-670-C: Administration of medications and related provisions – medication administration.

22VAC40-72-(6)-430-A – Uniform assessment instrument (UAI) – All residents of and applicants to ALF shall be assessed face-to-face using the UAI 16
22VAC40-72-(8)-840-I – General requirements – cleaning and hazardous material storage 16
22VAC40-72-(6)-440-E – Individualized service plans – signed and dated 15
22VAC40-72-(5)-350-A – Physical examination and report – within 30 days prior to the date of admission 13

22VAC40-72-(6)-550-F – Resident rights – reviewed annually with each resident or his legal representative or responsible individual as stipulated in subsection G of this section and each staff person 13
22VAC40-72-(6)-670-B – Storage of medications – resident may be permitted to keep medication in a secure place capable of self-administering per UAI 13
