Use this QR code to access the SMART Questionnaire

As the VDH Office of Licensure and Certification (OLC) has resumed normal survey activity over the past month, VHCA is launching an online version of the SMART Questionnaire, which allows you to provide feedback regarding the survey process to help with the data analysis we provide. The information obtained from this questionnaire is vital to our discussions with OLC.

Responses to the SMART Questionnaire will be kept confidential and only shared in aggregate summary. You can also use this QR code to access the questionnaire.

VHCA-VCAL’s Survey Management and Analysis Resource Tool (SMART), is used to analyze current trends and patterns in the nursing facility survey process which includes type of survey, length of survey, type of surveyors conducting the survey, survey supervisor, a list of questions related to the survey process, and the citation description and detailed descriptors for each F-Tag.

Normal survey activity includes annual recertification surveys, onsite revisit surveys, and non-immediate jeopardy complaint surveys. OLC is using state surveyors and contracted surveyors, Healthcare Management Solutions (HMS).
