Reminder: The next submission deadline for the Skilled Nursing Facility Quality Reporting Program (SNF QRP) is May 18.

• This includes MDS assessment data and COVID-19 vaccination coverage for health care personnel submitted to CMS via the CDC National Healthcare Safety Network (NHSN) for September 1 – December 31 (Q4) of calendar year (CY) 2025, along with the annual NHSN Influenza Vaccination among Healthcare Personnel measure data (for the 2025-2026 season).

All data must be submitted no later than 11:59 pm on May 18, 2026.

• The list of measures required for this deadline can be found on the CMS SNF QRP website. 

Action for facilities: It is recommended that providers run applicable iQIES/NHSN analysis reports prior to each reporting deadline, to ensure that all required data has been submitted. Recommended iQIES reports include:

• The SNF QRP Provider Threshold Report, and 

• The SNF QRP Review and Correct Report.

Remember: Swingtech sends informational messages to SNFs that are not meeting SNF QRP thresholds on a quarterly basis ahead of each submission deadline.
