Virginia’s Nursing Centers and Hospitals Partner to Reduce Readmissions

VHCA-VCAL has partnered with Virginia’s hospitals to discuss how to reduce preventable hospital readmissions. VHCA-VCAL and the Virginia Hospital and Healthcare Association (VHHA) will host three regional meetings in December to discuss how nursing centers and hospitals can work together to meet the shared quality goals of reducing readmissions and improving care transitions.

Please save the date for these upcoming sessions:  

Shared Learning Sessions
December 12 | Northern Virginia
December 13 | Richmond
December 14 | Lynchburg

You should make plans to attend if:

  • You are responsible for reducing readmissions at your organization
  • You have access to tracking, trending readmissions data
  • You have direct responsibility for “transitional care” or care over settings

Additional details, including registration instructions, will be provided in early November. There will be no cost to attend, but advance registration will be required.
