Reducing Preventable Rehospitalizations: INTERACT Training Sessions
VHCA-VCAL is launching an initiative to reduce preventable rehospitalizations by providing training on the INTERACT Quality Improvement Program. We will be hosting sessions in the five Virginia Health Planning Regions beginning on March 25-26 in Charlottesville. Additional sessions are planned for April through June in Fredericksburg, Roanoke, Richmond, and Williamsburg. The training is free for one representative from each nursing facility. Reducing Preventable Rehospitalizations-INTERACT Training Sessions
All nursing facilities and hospitals have been assigned a meeting location based on the Health Planning Regions defined by the Virginia Department of Health to ensure acute and post-acute providers within the same region are attending the training sessions at the same time to decrease obstacles during implementation. Upon registering online, you will be prompted to select your organization’s name. You will automatically be shown the training location that corresponds to your health planning region.
We encourage you to attend the session designated for your facility to enhance the relationships with other providers in your region.

Day 1
Who should attend?
- Hospital quality and safety leaders who are responsible for ensuring that hospital staff execute safe transitions in care and are committed to reducing hospital readmission rates.
- Nursing facility staff who are responsible for implementing quality improvement programs to reduce acute care transfers, and when necessary, safely and effectively send residents to the hospital to improve care transitions by reducing preventable rehospitalizations.
10:00 am – 12:00 pm: Hospital Leadership Training
Pathway Health Services will provide a presentation on the INTERACT® Tools and Resources and implementation strategies to coordinate communication and successful outcomes of quality and tips for reduction of avoidable readmissions between acute care and post-acute care facilities.
1:00 pm – 3:00 pm: Nursing Center Leadership Training
Pathway Health will provide an overview of INTERACT™ and operational strategies for nursing facility organizational leaders.
Is there a registration fee? There is no cost for certified nursing centers and hospital staff to attend one of the two sessions.
Day 2
Who should attend?
- Each licensed nursing facility may register one “readmissions champion” for its organization, who is leading the readmissions reduction efforts.
10:00 am – 3:00 pm: One-Day INTERACT Training
Pathway Health will provide a one-day interactive program which will provide facility leadership with an introduction, background, and overview of the purpose of the INTERACT™ QIP Program.
Is there a registration fee? Grant funding will cover the cost of one attendee per Virginia nursing facility. A facility may send additional attendees for a fee of $196 per person. Payment for the additional attendees is due at time of registration. Lunch will be provided on Day 2 by sponsors.
About the Initiative
VHCA-VCAL is proud to partner with Pathway Health and the Virginia Hospital and Healthcare Association (VHHA) to provide this training, which is funded by a grant from the Virginia Department of Medical Assistance Services using civil monetary penalty grant funds. The goal of this project is to achieve a combined 10% overall reduction in preventable rehospitalizations for individuals who reside in nursing facilities throughout Virginia by June 30, 2020.
