VDH has set up a form that nursing facilities and assisted living facilities can use to request a remote infection prevention and control assessment (teleICAR). Requests will be prioritized by facility type, outbreak status, recent onsite visits, and team availability.

Requests can be submitted by the facility or the health district on behalf of the facility. Assessments will be conducted by the VDH Healthcare-Associated Infections and Antimicrobial Resistance Program (HAI-AR Program).
