Overview

This 6-week masterclass led by experts from Proactive LTC Consulting is designed to equip skilled nursing facility (SNF) professionals with the knowledge and tools needed to drive Quality Measures (QMs) through the pursuit of clinical excellence. Each week, participants will delve into critical aspects of resident care that directly impact QMs, from comprehensive assessments and care planning to preventing adverse events and managing chronic conditions.

Through a combination of expert insights, evidence-based strategies, and real-world case studies, this series will empower healthcare providers to enhance clinical outcomes, reduce negative events, and elevate their facility’s QM performance. Participants will also learn how to integrate Quality Assurance and Performance Improvement (QAPI) processes, leverage data-driven decision-making, and sustain long-term improvements through continuous staff education and interdisciplinary collaboration.

 

Sessions

Each webinar will be held from 2:30 – 3:30 pm. Full session descriptions provided below.

  • May 28 | Introduction to Quality Measures (QM): Review the fundamentals of QM calculations, common challenges, and practical strategies to improve scores.
  • June 4 | Clinical Excellence in Care Planning and Assessment: Learn best practices for conducting accurate assessments, developing individualized care plans, and aligning clinical processes with QM goals.
  • June 11 | Preventing Adverse Events to Enhance QMs: Gain insight into common types of adverse events, including falls, medication errors, infections, and pressure ulcers, and their impact on quality metrics. This presentation will provide attendees with practical strategies to prevent adverse events and optimize care delivery.
  • June 18 | Managing Chronic Conditions: Discover how to identify and mitigate risk factors, optimize care plans, and leverage date-driven approaches to monitor and enhance outcomes, improve the quality of care, and enhance overall resident satisfaction, contributing to better quality measure performance.
  • June 25 | Enhancing Functional Outcomes: This course will provide strategies for enhancing functional outcomes through best practices, resident-centered care, and interdisciplinary collaboration.
  • July 2 | Sustaining Clinical Excellence for Long-Term Quality Measure Improvement: Explore practical strategies using a QAPI-driven approach to uphold lofty standards of quality care consistently. Participants will learn data-driven methods to set quality improvement goals, develop effective interventions to enhance quality of care, and achieve sustainable outcomes.
