The Virginia Hospital and Healthcare Association (VHHA) has scheduled a webinar on June 16 from 10:00 am – 11:30 am for an introduction to the Virginia Readmissions Reduction Initiative.

Why Is This Important to Skilled Nursing Facilities?

According to Medicare data, 44% of Medicare hospital readmissions are from a post-acute setting (SNF or Home Health).  As such, meaningful reductions in hospital readmission rates will necessarily involve collaboration with post-acute providers.  As Virginia’s hospitals explore strategies to reduce readmissions, skilled nursing facilities need to be at the table.
