Pilot Study on Transfer of Health Information and Care Preferences Quality Measures

Facilities that have been Medicare-certified for at least one year are eligible to participate in a pilot study on two Transfer of Health Information and Care Preferences (TOH) quality measures.  Responses are due by January 17.

CMS has contracted with RTI and Abt to develop and implement the TOH quality measures which meet requirements under the Improving Medicare Post-Acute Care Transformation Act (IMPACT Act) of 2014.  Data for these cross-setting measures will be collected for the following post-acute care settings:  SNFs, IRFs, LTCHs and HHAs. As part of the measure development process, RTI, in collaboration with Abt, is pilot testing the TOH measures to investigate data collection methods and time to complete the items, the feasibility of implementing the TOH quality measures, and measure properties.

CMS requests that interested IRFs, SNFs, LTCHs, and HHAs submit an Interest Form to sclark@rti.org by Tuesday, January 17. Interest forms received after January 17 are welcome and will be considered for the later testing activities.
