Free Resource: Advance Directive for Dementia
Dr. Barak Gaster, a primary care physician at the University of Washington School of Medicine, has developed a five-page dementia-specific advance directive to help individuals describe which interventions they would want, or not, during phases of their illness. The document is available online at no cost.
